      I paid Dawn Martin of Chatfield Legal Document Service in April 2018 to form an LLC for me and to provide me with the document I need to place my business into the LLC.

      The LLC was approved in September 2018. As of February 2019 she has not provided me with the last document I need.

      I have contacted her numerous times asking for this document. She has been full of these excuses:

      I thought I did this.
      I thought I told you that.
      I thought I emailed you that form.
      I did not have email or phone service due to a winter storm.
      I thought I mailed that form to you through USPS.
      I had a death in my family.
      I was in the hospital for a few weeks.

      I recommend you do NOT use Dawn Martin or Chatfield Legal Document Service. I am extremely dissatisfied with the service I received.
